New Partnership Expands Access, Bringing At-Home Care to 18,000 Arizona Families on Medicaid

Many families do not have easy access to care. Some may not have a car, live near a clinic, or have help with their kids. This new in-home option helps better meet that need by bringing medical, mental, and social care to their door.
Blue Cross Blue Shield of Arizona Health Choice has teamed up with Nest Health, a group that brings care straight to people’s homes. Together, they will offer families a new way to get help, whether for a doctor’s visit, a mental health check, or support with food, housing, and other needs.
“We want to help our members who do not have steady care or support,” said Dr. Heather Carter, Medicaid Segment General Manager at Blue Cross Blue Shield of Arizona and Health Choice CEO. “This program makes it easier for families to get the help they need, right where they are.”
Arizona is one of the hardest-hit states when it comes to finding enough providers. In rural areas like Mohave, it is even worse. Patients often must travel far or wait too long for care. This partnership with Nest Health solves that by sending care teams into families’ homes.
The Nest Health team of doctors, nurses, therapists, and family advocates provide in-home and online care, as well as ongoing care coordination and 24/7 clinical support. The team can give vaccines, order lab tests, help with school forms, manage long-term health problems, and provide therapy or mental health support. They also help families find resources for things like food, housing, and employment.

The program is now in Maricopa County and Mohave County, with plans to grow over time. Eligible families on Health Choice will be contacted about how to sign up.
