Your health plan changed. Here's how plan changes work in Arizona.

You still have AHCCCS (Arizona Medicaid), but your health plan is different now. Your plan decides which doctors and clinics you can see. Here's when you can change plans and how.

Step 1

Check which plan you have now

Log in to Health-e-Arizona Plus, check your newest AHCCCS card, or call AHCCCS Member Services at (800) 962-6690.

Step 2

Know when you're allowed to change

  • Your yearly enrollment time: Every year, you get a 1-month window to switch your health plan. It is based on the month you first joined AHCCCS or the month of the longest-enrolled family member in your house. AHCCCS will mail you a notice when your enrollment choice month arrives.

  • If you were assigned a plan you didn't choose: you can pick a different one within 90 days of being assigned. Look for a "Freedom of Choice" letter.

  • You can request an immediate plan change outside of your annual window anytime, if you meet one of these conditions:

    • Your family is in different plans: you can ask to move everyone into the same plan.

    • You moved to a county where your plan isn't offered.

    • American Indian and Alaska Native members can switch between the American Indian Health Program and a managed care plan at any time.

Step 3

Before you choose a health plan, check your doctors

All AHCCCS health plans cover the same medical services, but each has its own doctors and clinics. Ask whether your doctors are in a plan's network by calling that plan's member services.

Nest Health currently works with AHCCCS plan: Blue Cross Blue Shield of Arizona Health Choice.

Step 4

Make the change yourself

  • By Phone (Fastest): Call 1-800-334-5283 (Statewide) or 602-417-7100 (Maricopa County) to speak with a Beneficiary Support Specialist.

  • By Mail: Send a written request to AHCCCS, PO Box 25520, Phoenix, AZ 85002.

Only you (or your authorized representative) can change your plan; a doctor's office can't do it for you.
