Your health plan changed? Here's how plan changes work in Louisiana.

You still have Louisiana Medicaid, but your health plan is different now. Your plan decides which doctors and clinics you can see. Here's when you can change plans and how.

Step 1

Check which plan you have now

Log in at mymedicaid.la.gov, check your newest Medicaid card, or call Healthy Louisiana at 1-855-229-6848 (TTY 1-855-526-3346).

Step 2

Know when you're allowed to change

  • Any time, for any reason: since March 1, 2026, you can switch your health plan whenever you want. You don't need a reason.

  • Up to two changes a year: after your second change in a calendar year, you stay with that plan until January 1, unless Louisiana Medicaid approves a "for cause" reason (for example, you move out of the plan's area).

Plan details and enrollment information are at myplan.healthy.la.gov. For a paper copy by mail, call 1-855-229-6848.

Step 3

Before you choose a health plan, check your doctors

Ask whether your doctors, clinics and pharmacy are in a plan's network. Call that plan's member services or Healthy Louisiana.

Nest Health currently works with these Healthy Louisiana plans: AmeriHealth Caritas Louisiana and Aetna Better Health of Louisiana.

Step 4

Make the change yourself

Use the Healthy Louisiana app, go to myplan.healthy.la.gov, or call 1-855-229-6848. Only you (or your authorized representative) can change your plan; a doctor's office can't do it for you.

Your new plan usually starts on the 1st of the next month. Until then, keep using your current plan’s card for visits and prescriptions.

Step 5
