Lost your Louisiana Medicaid? Here’s how to get it back.
Many families lose Medicaid because of a missed letter or a paperwork problem, not because they stopped qualifying. These steps can help you get coverage back, often without starting over.
Step 1
Find out why your coverage ended
Look for a letter from Louisiana Medicaid. It says why your coverage stopped and what you can do. Can't find it? Call Louisiana Medicaid at 1-888-342-6207, Monday to Friday, 8 a.m. to 4:30 p.m.
Step 2
If you missed your renewal or had trouble finishing it, renew now
It’s not too late if your coverage ended within the last 90 days. Do not fill out a new application; instead, simply turn in your completed renewal packet right away. The sooner you do, the faster your coverage will be restored.
Pick the way that's easiest for you:
Online (fastest): log in at mymedicaid.la.gov and choose to renew your coverage.
By phone: call 1-888-342-6207.
By email, fax or mail: send your renewal form to MyMedicaid@la.gov, fax 1-877-523-2987, or mail Louisiana Medicaid/LaCHIP, P.O. Box 91283, Baton Rouge, LA 70821-9278.
In person: visit a regional Medicaid office, Monday to Friday, 8 a.m. to 4:30 p.m.
Step 3
If you renewed and were denied but think you still qualify, you can appeal
You have the right to ask for a fair hearing. Your denial letter explains how and gives a deadline, so act quickly. Ask if you can keep your coverage while you wait for a decision. Free civil legal aid and public benefits navigation may be available through regional organizations like Acadiana Legal Service Corporation and LouisianaLawHelp.org.
Step 4
Have these ready
Social Security numbers, pay stubs or other income information for everyone in your home, and information about any other health insurance you have or could get through work.
Step 5
Keep your address and phone number up to date with Louisiana Medicaid
Medicaid sends renewal notices by mail. Update your information at healthy.la.gov or by calling 1-888-342-6207, so you don't miss the next one.
Step 6

We'll check whether we can restart your care and set up a visit. We'd be glad to welcome you back.
