If you're moving your care to a new provider, we're glad to send your records over — or speak with your new provider directly about your care. Just fill out this form or email records@nesthealth.com to authorize the transfer.
What to have ready
The patient's full name and date of birth
A picture of your government-issued photo ID
If you’re requesting on behalf of the patient, proof of legal authority to act for the patient
E.g., birth certificate, custody/guardianship order, power of attorney, letters testamentary
Your new provider or clinic's name, and their fax, email, or mailing address
Your information is only used to process this request and will be handled in line with Nest Health's privacy practices.
What happens next
Once we receive your signed request, we'll send your records to the provider you listed. If you'd like, we can also speak with your new provider directly about your care history — just note that on the form.
