  • Who will come to my home?

    In-home visits are conducted by two members of the care team, your provider, and a medical assistant. Our in-home providers are experienced nurse practitioners and our family advocates are highly-trained medical assistants. Your Nest team may also include family doctors, nurses, and mental health specialists to care for you and your family by phone and video when needed.

  • How much does Nest cost?

    There is no cost to Blue Cross Blue Shield of Arizona Health Choice members.

  • Does Nest replace my OB/GYN?

    No, Nest is a primary care provider and will not replace your OB/GYN. We partner with your other doctors to coordinate care.

  • Who does Nest take care of?

    Your Nest team provides care to parents under the age of 65 and their children, ages 0-18 who live in the same household. We offer primary care, mental and behavioral health care, access to specialists, nutritional support, and more.
