Lost your AHCCCS coverage? Here's how to get it back.

Many families lose AHCCCS (Arizona Medicaid) because of a missed letter or a paperwork problem, not because they stopped qualifying. Acting quickly matters: if you get coverage back within 90 days, AHCCCS puts you back in the same health plan you had before.

Step 1

Find out why your coverage ended

Log in to Health-e-Arizona Plus to see your letters, or call AHCCCS Member Services at (800) 962-6690.

Step 2

If you missed your renewal or had trouble finishing it, renew now

It’s not too late if your coverage ended within the last 90 days. Do not fill out a new application; instead, simply turn in your completed renewal packet right away. The sooner you do, the faster your coverage will be restored.

Pick the way that's easiest for you:

  • Online (fastest): log in to Health-e-Arizona Plus to complete your renewal, upload documents, or check your status.

  • By phone: call 1-855-HEA-PLUS (1-855-432-7587), Monday to Friday, 7 a.m. to 6 p.m.

  • In person: visit a DES office and bring your renewal letter and any documents it asks for.

Step 3

Get free help filling out forms

Call the Health Choice Community Assistor Program at (844) 390-8935. They can help you reapply or fill out forms at no cost.

Step 4

If you renewed and were denied but think you still qualify, you can appeal

You have the right to ask for a fair hearing. Your denial letter explains how and gives a deadline, so act quickly. Ask if you can keep your coverage while you wait for a decision. Free civil legal aid and public benefits navigation may be available through regional organizations like Community Legal Services (CLS), Southern Arizona Legal Aid (SALA), DNA People's Legal Services, and AZLawHelp.org.

Step 5

Keep your address and phone number up to date with AHCCCS

AHCCCS sends renewal notices by mail. Update them in Health-e-Arizona Plus or by calling 1-855-432-7587, so you don't miss the next one.

Step 6

Got your coverage back? Call us

We'll check whether we can restart your care and set up a visit. We'd be glad to welcome you back.
